The last segment was just heartbreaking. I love the detail of Dean leaving Sam written instructions on how to take care of his "baby"
I wish I could say leaving a list of instructions was original, but there are other fics with the same idea. I can't see him leaving Sam the car *without* such a document, though.
And I'm sorry about the hearty-breaky-thing. I myself usually avoid "Dean doesn't get out of the deal" fics because NOOOOO. They just can't kill him. It will break MANY hearts.
